Our comprehensive care model includes considering all aspects of the patients' needs—family support, comprehensive medical evaluation and nutrition, sleep, epilepsy, ADHD, therapies, and long-term financial planning (such as transitioning to adulthood, ABLE accounts, guardianship, and special needs trusts).

Appointment Information


The Initial Visit:

Our team will do whatever it takes to make every visit to our office a pleasant and relaxed experience. Please allow sufficient travel time to arrive 5-10 minutes early. With all of the information about our practice available on our website, you can feel confident that you and your child are well-prepared for your first appointment. Please provide the following information when you arrive for your visit:

  • Any previous Evaluations and/or Assessments (feel free to fax these reports to our office prior to the appointment)

  • Fill out our new patient forms

  • Children with neurological conditions require a comprehensive evaluation to identify potential causes and develop an effective treatment plan. Unfortunately, insurance companies limit the time available for patient visits, resulting in fragmented and incomplete care. By choosing to operate outside of these constraints, we can provide a more personalized, thorough approach to treatment.

    Our decision to operate out of network allows us to dedicate more time to each patient and reduces the volume of cases we handle. While this enables us to offer higher-quality care, we can still provide you with the necessary documentation for Out-of-Network benefits, if needed. We accept payment via cash, credit card, CareCredit, and Health Savings Account (HSA) cards at the time of the visit.

  • Yes, We will use your insurance when ordering any medical tests or procedures.  Coverage for tests/procedures will be determined based on your insurance plan.

  • No we do not require a referral but strongly recommend you discuss your concerns with your pediatrician to make sure that the evaluation is appropriate for your child.

  • Yes, a physician will see you every visit. 

  • No, we see children and adolescents with any type of neurological concern. Dr. Fields has extensive experience treating children with epilepsy, migraines/headaches, tics, hypotonia, muscle weakness, traumatic brain injuries, genetic disorders, and more.

  • At least 48 hours' notice is required to reschedule the appointment. A call on the same day cancelling the appointment will be treated as a missed appointment. If a new appointment is missed, our scheduling coordinator will do their best to re-schedule you. A $100 charge applies for a missed appointment.

  • We accept payment via cash, credit card, CareCredit, and Health Savings Account (HSA) cards at the time of the visit.
